Psych Home Care

The Psych Home Care service offers 6-12 home visits or visits to community locations (e.g. cafe or walking) delivered by a psychologist, social worker or registered nurse for the purpose of supporting patients after discharge or as an alternative to admission. The aim of these sessions is to offer psychoeducation and support to implement skills for distress tolerance, behavioural activation, self-care and medication management to enhance the patient’s capacity to manage their symptoms at home to reduce the requirement for repeated inpatient admissions.
